Personal Care AssistanT/Caregiver

description

  3. Ensuring all case files, and other records, strictly comply with policies, regulations, and procedures.

  4. Assess and correct safety risks in homes to provide a safe environment and facilitate mobility.

  5. Attends case conferences. 

  6. Consults with the physician and other members of the health team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient’s health problems.

  7. Participates in staff development and revision of the physician’s plan of treatment.

  8. Conduct patient/client self-assessments.

  9. Participates in discharge planning and in-service programs..

Registered Nurse

  4. Create treatment and therapy plans to suit the individualized needs of a diverse population of patients.

  7. Educate patients and their families regarding cognitive, communication and swallowing disorders.
