Please fill out the form (which is sent immediately after purchase), and return the document via email (oana@oanahealthcare.com). The name must match with payment on file. There are no refunds for organizational charts.   
 
Orders are not processed on weekends or holidays. We will process your order, the following business day.  Please allow 24-48 hours to receive an organizational chart.
